Risk Assessment Questionnaire No Longer Required
Applicants are no longer required to submit the CDC Risk Assessment Questionnaire with their Notice of Funding Opportunity (NOFO) application. CDC continues to conduct the review of risk as described in the Review and Selection Process.
NIOSH may ask for additional information prior to the award based on the results of the review. Submitted Risk Questionnaires will not be included in this risk review. (January 2024)
